On-Call Nurse Manager (Hospice) - Afternoons / Evenings
An Upstate New York healthcare organization is currently seeking an experienced, licensed Registered Nurse (RN) to join their Afternoon / Evening shift as their new On-Call Nurse Manager (Hospice). In this role, the On-Call Nurse Manager (Hospice) will be responsible for providing overall supervision of the clinical team and coordination of services after hours and on weekends for patients in hospice homecare, skilled nursing and assisted living facilities.

- Hiring, mentoring, evaluating and separating direct reports
- Participating in the orientation, mentorship and staff development of after-hours clinical staff in collaboration with the education department to assure staff is current in clinical information and practice
- Maintaining a strong working relationship with all clinical team Managers and Directors to ensure seamless communication and coordination of patient care and services
- Overseeing the scheduling of staff in collaboration with the managers/directors and maintains appropriate staffing levels to meet patient/family needs after-hours
- Provides guidance and support in the planning of patient care while maintaining a leadership role for all members of the on call team
- Ensuring timely response and appropriate triaging of all phone calls and visits by providing a regular presence on the after-hours shifts and weekends, field supervisions and chart reviews
- Ensuring care is implemented that meets the physical, psycho-social and spiritual needs of the patient/family while maintaining standards of care consistent with the Hospice.
- Participating in the implementation of agency wide Continuous Quality Improvement initiatives and assisting in multiple monitoring activities that are both patient and organizational focused to promote clinical quality and corporate compliance
- Providing follow up for any patient/family complaints related to on call services in collaboration with the patient advocate with staff coaching and corrective action, as appropriate
- Performing other duties, as needed
- 2+ years of Clinical Nursing experience
- Graduate of an accredited School of Nursing
- Current NYS Registered Nurse (RN) licensure
- Excellent knowledge of Electronic Medical Records System
- Microsoft Office Suite proficient (Excel, Outlook, PowerPoint, Word, etc.
- Solid assessment and documentation skills
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
- Bachelor's Degree in Nursing
- Certification in Hospice and Palliative Care Nursing
