Bloom Healthcare is adding a Chronic Care Coordinator to our team! We’re looking for passionate individuals committed to improving the lives of homebound seniors. This position will be responsible for completing Chronic Care Management tasks for our patients and providers. The position collaborates with our clinical team, providers, intake and our community care partners.
The CCC RN responsibilities include:
- Updating patient care plans
- Preparing patients and providers for upcoming visits
- Reconciling medications
- Coordinating care with community partners
- Coordinating post-hospital plans of care
- Collaborating with the Interdisciplinary Care Team (IDC)
CCC RN Knowledge/Skills/Abilities:
- Strong computer skills
- Ability to multi-task at a very high level
- Strong problem solving abilities
- Excellent phone/customer service skills
- Familiarity with EMR software a plus
Required:
- Current Registered Nurse (RN)
