Registered Nurse (RN)
4 Corner Resources is currently searching for Registered Nurses (RN) to perform assessments in patient's homes around the New York, NY area. This is a contract with a large healthcare organization that is looking to add new members to their team as soon as possible. You will receive a mileage percent for your traveling starting from the first location. There are five boroughs in the area that require coverage. Registered Nurse (RN) Details: Multiple positions available Duration: 3-6 months - Position may convert to permanent employment Schedule: 8:30am - 5:30pm, Monday through Friday. 40 hour work week 2-3 visits daily. Each visit lasts about 2 hours Paid 4 week training Laptop and any equipment needed for the position will be provided by the organization After 3-6 months as a contract employee, there is potential for conversion to permanent employment! If offered, the new hire will be eligible for a $10k lump sum sign-on bonus that will be paid out after 3 months (90 days).
Duties and Responsibilities: Registered Nurse (RN) Responsibilities: Facilitate home assessments for prospective members and re-assessments of current members using the Uniform Assessment System (UAS) Create enrollment and continued eligibility determinations by utilizing clinical expertise and critical thinking skills. As needed, conduct assessments in settings other than home as needed (ex. skilled nursing facilities) Provide feedback to home care agencies and CMT regarding home care issues and the aides performance Review previous nursing assessments/medical notes to determine necessary adjustments or updates in Care Plan and/or to assist with nursing home admission Offer community-based training for frail population care including self-care techniques and prevention strategies Ensure prospective and current members and significant others/responsible parties understand and are in agreement with enrollment in a managed long-term care plan Registered Nurse (RN) Qualification Requirements: New York State RN license. Ability to travel within your choice of 2-3 locations that include: Bronx, Manhattan, Staten Island, Queens, Brooklyn, Long Island, or Westchester Must be vaccinated against COVID-19 (this includes 1 booster if due).
